From 1 January 2026, Sunbury and Cobaw Community Health (SCCH) and Nexus Primary Health (Nexus) will come together under a new name – Omnia Community Health. This marks an exciting new chapter, bringing together the expertise, services and support of Nexus and SCCH to better serve our communities.

Our purpose is simple: to help our communities feel healthy, connected and supported through trusted, local care. Our aim is to be the trusted local health partner for our communities – one that empowers people to live well through care that is inclusive, connected and grounded in local relationships.

For the time being its business as usual, and you will continue to receive your services and care from Nexus and SCCH.

We are looking forward to coming together as Omnia Community Health on 1 January 2026 and will be developing this website to incorporate all of our services. In the meantime, you will continue to find information on the Nexus and SCCH websites and on our social media channels.
